INFORMATION & CONSENT FORM
Dear all,
Please fill in the consent form below, acknowledging that you have read the information and
are happy for your child to attend these classes online.
This is required by our governing body (CLRG) and our insurance company. Failure to return the consent form will result in your child not being covered by the class insurance policy in the event of accident/injury.
Key points to remember
*Please ensure that your child dances on a suitable surface in an area that is clutter free.
*All cables/wires must be secured.
*Appropriate footwear must be worn- socks are not appropriate.
*Suitable apparel to be worn, as one would at normal class.
*Children should warm up and cool down as at regular classes.
*We will issue technical instruction but as always the health and safety of our dancers is our primary
concern and if you have any concerns, please contact us.
